Botox

Botox®, Dysport®, Nucieva® and Xeomin® are brand names for Botulinum toxin. They are also called neuromodulators. Botulinum toxin has been used in cosmetic procedures since 1987. It is an effective and safe way to slow down the aging process by temporarily relaxing overworked facial muscles that cause lines and wrinkles. Neuromodulators are approved by Health Canada and the FDA for cosmetic use. Tiny needles are used to inject small amount of toxin in facial muscle which will result in temporary relaxation of that muscle. The skin over that muscle will not get pulled any more so no lines will be formed. Over the time though, if not treated, fine lines will become deeper creases in a skin and will form static lines that will not go away even if you are relaxed. The good news are that you can also treat those with neuromodulators. You would need to come for regular treatments just before old toxin will wear off from your body so there will be no muscle activity. Overtime the static line will become softer creating fresh and youthful appearance. Combination treatments should be used in these cases to treat the skin also which will expedite the process to smoother and glowing skin. In addition other contributing factors needed to be considered when assessing the wrinkles such as sun damage, genetics, a poor diet, or smoking.

When and how long the effects will last?

The onset of effect will be in 2-4 days however the full results will be seen in 2 weeks post injection. Depending on individuals’ metabolism and other factors as well as areas treated, the effect will last from three to six months.

How to prepare for an injection?

Refrain from drinking alcohol and taking blood thinning medications and supplemets such as Advil, Aspirin, Aleve, Motrin, Fish oil, Omega-3, Ginko biloba, Vitamin E and garlic for a few days prior to your appointment. We will also ask you to avoid exposure to heat such as saunas and hot tubs post treatment so you should be prepared to make an arrangements.

Can I go back to work afterward?

Yes, after your appointment you can return to your normal activities. You would only need to stay upright for four hours post injections. You would also have a small mosquito like bumps in the spots of injections which will go away in less than an hour.

What is Hyperhidrosis?

While sweating is good for the body, hyperhidrosis, or excessive sweating, is not. Hyperhidrosis occurs when you sweat outside the normal needs of the body, such as when you sweat when it is not too hot. When you sweat excessively at night, when your clothes get soaked in sweat, when sweat drips off your body, and when sweat cannot be regulated with antiperspirants, you probably have hyperhidrosis. Hyperhidrosis usually occurs in one area of the body, the most common parts being the feet, hands, armpits, and forehead. 

What Causes Hyperhidrosis?

There are two types of hyperhidrosis – primary and secondary hyperhidrosis. 

• Primary hyperhidrosis causes excessive sweating on both sides of the body. There’s no known reason for this type of abnormal sweating. It is believed family history plays a role in primary hyperhidrosis. It usually starts in childhood or teenage years. 
• Secondary hyperhidrosis, on the other hand, is caused by underlying medical conditions such as cardiovascular and respiratory disorders, endocrine or metabolic disorders, neurological conditions, infection, menopause, cancer, or hyperthyroidism. 

Hyperhidrosis Treatment

Given that secondary hyperhidrosis is caused by underlying medical conditions, treatment involves treating the underlying medical condition. The first instance of primary hyperhidrosis is usually treated through topical application of aluminum chloride, which is found in most antiperspirants, but most people with excessive perspiration do not respond to topical treatments. 

Anticholinergic medications are also used to treat primary hyperhidrosis. These work on the automatic nervous system to reduce sweating, but they can cause blurred vision, constipation, urinary retention problems, and dry mouth. 

Anti-wrinkle Injections to Treat Hyperhidrosis

The most common hyperhidrosis treatment today is anti-wrinkle injections. Anti-wrinkle injections, or neuromodulators, temporarily paralyze the nerve signals that go to the sweat glands in the area they are injected. Since hyperhidrosis is caused by overactive nerves activating the sweat glands, neuromodulators are effective in reducing hyperhidrosis in localized areas such as the face, scalp, underarms, feet, and palms.

Anti-wrinkle injections do not have permanent side effects, and you will not experience any downtime. You simply need to walk in for the procedure, and you will be done in about 20 minutes. It takes about 7 days to start noticing results and about 2 weeks for complete dryness. Note the treatment results lasts about 18 months for the face and armpits and about 6 months for the hands and feet.

Who is a good candidate for lip flip?

What Causes a Droopy Nose?

Those with overly strong septi muscle have a droopy nasal tip. The depressor septi muscle is the muscle that connects the upper lip to the central base of the nose. As we age, the shape of the eye sockets deepens, making the nasal bridge appear more prominent, and the eyes appear close together. Lowering the height of the nasal dorsum can rejuvenate the face. The falling of the tip also causes the nostrils to start widening and the nasal dorsum to be more prominent, which can affect the entire balance of the face.

Surgical Nose Tip Lift Treatment

Correction of this deformity is challenging. Plastic surgeons reduce the depressor septi muscle in a procedure called closed (endonasal) rhinoplasty. With this procedure, an external incision is not necessary, and the muscle can be addressed through incisions hidden in the gum line inside the upper lip or internal nasal incisions. 

The reduction, cutting, or detachment of the depressor septi muscle can be performed as part of an entire rhinoplasty (such as hump reduction) or in isolation. The droopy nasal tip may require additional operations like cartilage reshaping, sutures, and cartilage grafts. Being a surgical procedure, there are some risks with rhinoplasty. These include bleeding, infection, adverse reaction to anesthesia, difficulty breathing through the nose, permanent numbness around and in the nose, scarring, septal perforation, pain, discoloration and or swelling, and an uneven-looking nose. So, what is the alternative to rhinoplasty?

Non-Surgical Nose Tip Lift Treatment

Neuromodulators are the nose-shaping solutions you need to lift your nose to where it belongs. This treatment option is less invasive and may give you results that are just as good as rhinoplasty. 

Despite popular belief, neuromodulators do not lift up the nose. Rather, they relax the depressor septi nasi muscle that is responsible for pulling the nasal tip downwards. Neuromodulator injections are done at the base of the nose to prevent the tip from turning down. A neuromodulator injection can also be done on the upper lip muscle to prevent gum exposure and create a smile that is less gummy. Injection with neuromodulators is a quick office procedure. It takes between one and two weeks for the results to be apparent. The results will last three to four months. 

How does nose slimming work?

A small amount of neuromodulator is injected with tiny needle to the sides of nostrils. The muscle called Alar Nasalis will become limited in its activity which will create a slimmer look of nose. The nostrils will not be expanding too widely with facial expressions. There will a slimming effect created from both sides of nostrils.

Results

Results can be seen in 10 to 14 days and the effect will last for about 4 to 6 months.

How does it work?

Tiny muscles in skin that are called pili muscle are responsible for contraction and pushing sebaceous oil into pores. Neuromodulators can be applied by two different ways to improve skin quality and reduce appearance of enlarged pores.

1. Superficial intradermal injections. Neuromodulator can be hyper diluted diluted with mesotherapy cocktail or just saline and injected very superficially using tiny needles. It will allow for product to stay on very top layer of the skin where it should work. Hyper diluted neuromodulator will not be able to pass through and affect muscles that are responsible for facial movement so your facial expressions will not be affected with this technique.
2. Microneedling. Diluted neuromodulator can be applied during your microneedling session on your areas of concern. It will get absorbed by open channels from microneedling and will get delivered to dermal layer of skin. Micronnedling itself has plenty of benefits such as tightened and brightened skin, even tone and texture, improved appearance of fine lines and pigmentation. Combination of these two treatments will provide outstanding results.

What to expect

Following your treatment you may have some minimal redness from injection points that will fade away by the end of the day. You may begin to see the effect in 3 to 5 days. The skin will be smoother and pores will be decreased in size. Some people notice also decrease number of breakout periods.

The results generally last for up to 4 month. We recommend to return after that period for maintenance treatment.

Cauliflower / Dimpled Chin Treatment

The most common chin problems are double chins, small chins, receding chins, chins that are out of proportion to the rest of the face, weak chin/lack of proper jawline definition, and chin dimpling or wrinkling. There are different causes for these chin issues. As an example, double and receding chins are a result of ageing. Those with an overbite are more susceptible to weak chins and double chins. 

Cauliflower or dimpled chins and other chin problems are caused by the size, relative tension, and position of the mentalis muscle. This is a muscle in the chin that extends from the crease under the lower lip to the bottom of the central chin. The muscle is a paired group – there are two muscles on either side of the chin with a gap in the middle. The muscle is responsible for elevating the lower lip. 

What is a Dimpled Chin and What Causes It?

Dimpled chin, also called peau d’orange or “orange peel skin” is a condition where the skin has the dimpled texture of cauliflower or an orange peel. The dimples on the chin cannot be easily covered with makeup and other cosmetic products.

What Causes Dimpled Chin?

A dimpled chin is caused by the straining of the mentalis muscle. The muscle is hyperactive, and many people are unaware they are constantly contracting it, such as when focusing or concentrating on a task. The continuous stretching of the skin is what leads to the dimpled appearance.  Patients born with an overbite or a weak chin may strain the muscle and often have a hard time keeping their lips open. Other than genetic reasons, failure of the mentalis muscle to attach to the lower chin board could also be due to prior jaw surgery that leaves the muscle at a higher position than it previously was. High-riding mentalis muscles have a ball-like appearance when viewed from the front and a blunted appearance when viewed in profile. 

Dimpled Chin Treatment Using Neuromodulators

Neuromodulators are one of the most common treatment options for cauliflower/dimpled chins. They work by smoothing the skin. Neuromodulators are basically “anti-wrinkle” injections that are usually used for wrinkled foreheads, but they are also effective on the chin. If the dimpled chin is a result of chin surgery, the injection can still be given after the surgery. Neuromodulators work by blocking the signal from the nerve that tells the muscles to contract, essentially freezing the muscle. This relaxes the muscle. 

The procedure can be completed in a matter of minutes. It starts with the cleaning of the area to prepare for the injection, marking the area where the needle will be inserted, injection, and then application of pressure on the injected area. You may feel some discomfort and may get some temporary bruising, soreness, or redness in the area, but this should fade quickly. Results usually come quickly, but it may take up to four to seven days to start seeing results. Other than relieving dimpling, neuromodulators also improve the jawline contour and complement the lower lip.

Bruxism or Teeth Clenching

Bruxism is a condition characterized by grinding and clenching the teeth while you sleep. However, some people experience bruxism even while awake. The leading causes of teeth grinding include stress/anxiety, crooked or missing teeth, an abnormal bite, or sleep disorders like sleep apnea. People who experience sleep bruxism must wear a mouth guard while sleeping to reduce the damage to the teeth and jaw. Recent research indicates that Botox is effective in reducing the symptoms and discomfort of teeth grinding. Botox is typically known for easing the appearance of wrinkles. However, it also helps relax any muscles it is injected into.

After years of sleep bruxism, the masseter muscles could be enlarged, resulting in an appearance of a strong, square jaw. Particularly women dislike this appearance because it detracts from a feminine look. Botox injections shrink the masseter muscles and soften the jawline.

After a Botox Injection

If you experience bruising or swelling after a Botox injection, apply an icepack at the injection site for 10-20 minutes. You should ensure that you adhere to all doctor’s instructions after receiving a Botox injection. For example, you should wait a minimum of four hours before lying down. You should not engage in strenuous activity for the first 24 hours after the injection. If you fail to follow the doctor’s instructions, Botox could migrate to other parts of the face, rendering the procedure ineffective.

The effects of the treatment will wear off after several months. Therefore, you will have to undergo additional treatment.

Teeth Grinding Will Stop Gradually

You might not notice the difference in the first few weeks after a jaw Botox. You might still grind and clench your teeth and even get tension headaches. Note that:

• When Botox is injected to reduce fine lines and relax facial muscles, the effects are seen within 7-10 days
• When used for TMD and pain associated with teeth grinding, effects are seen within one week
• When Botox is used to reduce masseter muscle hypertrophy, the effect is seen within six weeks to three months

After treatment, you will find yourself having to actively relax your muscles less and less. The headaches will also stop.
